General Data Protection Regulations (GDPR), Information, and Consent

As a provider of Medical Services, both Jenna Sherratt and Shelley Robinson are required to record details of all patients they treat. This includes medical information (including medication) and medical history, along with your general demographic details.

We are required to retain medical records for a minimum of 7 years after the last full year since treatment. (Timescales may vary due to circumstances such as age at first treatment and mental health capacity).

Your right to be forgotten cannot over ride the requirement for medical notes to be held for the above mentioned minimum retention period of at least 7 years.

We do not use your information for any other purpose than providing Health and Medical services, and where required, we may share this information with other medical professionals, such as your G.P, to assist in your care.

Your demographic information may be used to aid with appointment booking, and booking reminders.

Your information will not be sold to any third party.

You have the right to request a copy of any information we hold about you. This can be obtained by submitting a written request.

If you are not happy for this information to be retained, we will be unable to provide you with any Health or Medical care.

For those unable to consent to this information being retained the following applies:

  1. Permission from a parent or someone holding parental responsibility is required for all patients under the age of 16.

  2. In all instances where an Adults with Incapacity Document (A.W.I. Section 47)) is in place, this will require to be viewed and copied before treatment. In order that we can provide treatment, the A.W.I. Section 47 form must include specific reference to allowing treatment for Podiatry or by Allied Health professionals (A.H.Ps). If the A.W.I does not specifically state the above, a new updated version must be provided before any treatment can commence.
